Practice Policies and Information

  • Dr. Pokorny is not able to accept insurance and is an out-of-network provider with all major insurance companies. Payment for services is accepted via all major credit cards and/or HSA or FSA card. An invoice, known as a superbill, may be provided upon request to submit to insurance for partial out-of-network reimbursement. To know if you are eligible for out-of-network benefits, check with your insurance plan to see what your plan’s out-of-network benefits may be and if there is an out-of-network deductible.

    For information on fees for services, please see services. A form of payment is required to be on file to keep a scheduled appointment.

    There are no additional fees for routine medication refills, prior authorizations, or portal messages in between appointments. If an administrative task such as paperwork, insurance appeals, care coordination, etc. takes longer than 15 minutes an administrative fee may apply at a rate of $75 per 15 minutes.

  • It is requested that you provide at least 24 hour notice to cancel or reschedule an appointment. If appointments are missed or cancelled with short notice a no-show fee of 50% of the appointment rate may apply.

  • Patients may be seen at a variable appointment frequency that reflects their need for care. Patients needing more support may be seen weekly, bi-monthly, or monthly, while those with less needs may be seen once every 3 or 6 months.
